What should a dental enquiry form ask for?

The treatment the patient is asking about, and what they want in their own words. The form should attach the landing page and the channel itself, so nobody has to ring to find out what the enquiry even was.

Can it tell me which ad produced the patient?

It attaches the landing page every time, and the traffic channel where that data is available. So you can see which pages and channels bring patients.

Does the form book the appointment?

Not on its own. Booking straight into your practice management system is SmileBridge™, a premium add-on that sits on this same form. If your PMS gives you an API, then you can also book your patients directly.

Will a short form give us unqualified enquiries?

It gives you the treatment, the page and the patient's own message. That is more qualification than most practices get from a long form, because long forms are abandoned by exactly the patients you want.

Will it match our website?

Yes. It is a plugin your designer can style using CSS, so it carries your fonts, your colours and your buttons on your own domain.
